Concern

Fine lines

Lines print where collagen thins. What slows that, and the measured results of what rebuilds it.

Also called Early wrinkles, Crow's feet, Photoaging, Prejuvenation.

What it is

Lines start as mechanics. Your face folds along the same axes thousands of times a day, and young dermis springs back because it is dense with collagen and elastin. UV degrades both faster than age alone does, so the creases begin to print: first at the outer eyes where skin is thinnest, then the forehead and around the mouth. A fine line is a fold that has started to persist at rest.

The case for prejuvenation is arithmetic: collagen you keep costs less than collagen you rebuild. The evidence-heavy core is unglamorous, daily sunscreen plus a prescription retinoid, and tretinoin has decades of photoaging data behind it. Procedures add stimulus on top: controlled micro-injury in the dermis pushes fibroblasts to lay down new collagen, and that remodeling builds for 3 to 6 months after a session, which is why results are judged in months, not days.

The numbers, since promises are worthless: RF microneedling cut periorbital wrinkle scores by a mean 49% over 4 sessions in a 24-subject trial spanning Fitzpatrick II to V. A low-fluence 2910 nm laser facial improved overall photoaging 25.1% after 2 sessions, with pain rated 1.9 out of 10 and no numbing needed. Deeper resurfacing trades real downtime for larger change in fewer sessions. None of it stops aging; it moves your baseline.

What you may be noticing

  • Crow's feet that stay for a beat after you stop smiling
  • A first horizontal forehead line showing in flat light
  • Sunscreen or makeup settling into lines by afternoon
  • Fine crepe under the eyes on camera that you do not see in the mirror
  • Skin that still looks smooth at rest but creases sharply in expressions

How we approach it

  1. 1

    Photos and intake, online

You upload photos in even light and complete a structured intake before anything is booked.

  2. 2

    Phone consult

You talk through your goals, sun history and current routine with the practice by phone.

  3. 3

    Written plan, exact prices

Dr. Mueller reviews everything and you get options in writing, retinoid-only up to a device series, each with its itemized price.

  4. 4

    In-person exam, first visit

California requires a synchronous in-person exam before any laser or RF device is used, so it happens at your first visit and costs $0.

  5. 5

    Same-day first session

If the exam confirms the plan, your first treatment happens in the same visit, and the exam covers you for a year of treatments.

The evidence

  • RF microneedling for periorbital wrinkles: mean Lemperle score fell 13.00 to 6.09, a 49% reduction, across 4 sessions in 24 subjects, Fitzpatrick II to V (pubmed.ncbi.nlm.nih.gov/39418172)
  • 2910 nm fiber laser, superficial low-fluence mode: 25.1% mean photoaging improvement after 2 sessions in 20 subjects, pain 1.9 of 10, histology back to baseline at 2 weeks (pubmed.ncbi.nlm.nih.gov/36465001)
  • Fractional RF cohort: the share of patients at 25% or better improvement rose from 73% at 1 month to 88% at 6 months, so results build after the last session (pmc.ncbi.nlm.nih.gov/articles/PMC12471114)
  • Tretinoin: decades of published photoaging evidence on top of its acne labeling (drugs.com/pro/tretinoin.html)

Straight answers

Am I too young to start?
If lines are printing, biology has already voted; prejuvenation just means opening with the light tools (sunscreen, a retinoid, an occasional low-fluence laser facial) rather than waiting until deeper resurfacing is the conversation, and nobody here will sell you a series you do not need.
What does it cost?
A laser facial is $395 single or $1,050 for 3, RF microneedling is $425 per session or $1,140 for 3, and a retinoid prescription visit is $79; every figure is itemized on the pricing page and the first-visit exam is $0.
Retinoid first or procedure first?
Retinoid first in almost every case: it is the cheapest intervention with the deepest evidence, and 12 weeks of photos will tell you whether you still want a device series on top.
Does it hurt?
The low-fluence laser facial averaged 1.9 out of 10 without numbing in its trial; RF microneedling runs higher, around 5 to 6 out of 10 in published cohorts, so it is done with topical numbing here.
How long do results last?
Collagen you build is real tissue, not a coating, but aging keeps subtracting; trial results peak around 3 to 6 months and taper slowly, so most people plan periodic upkeep rather than a one-time fix.
I'm on isotretinoin. Do I have to wait?
Mostly no: a 2024 systematic review found no justification for delaying non-ablative lasers or RF microneedling during or right after isotretinoin, and the 2017 ASDS consensus agrees; fully ablative resurfacing is the exception and waits about 6 months, with Dr. Mueller making the call at your exam.
What about Botox?
Neurotoxin relaxes the muscle that folds the skin, a different mechanism for a different problem, and SkinMax does not offer it; the tools here rebuild the dermis under the line, which is the part that shows at rest.
